Step 2: Complete Your Online Application and Payment
Visit our secure portal and fill out the patient enrollment/re-enrollment form.
Pay the required fee: $130 for new certifications or $79 for renewals. This payment covers the provider consultation and our assistance with the DOH submission.
Step 3: Private Telehealth Consultation with a New Mexico Practitioner
You’ll be connected to a licensed Doctor for a private, virtual consultation via telemedicine.
She practitioner will review your submitted medical records and discuss your condition to issue a Medical Provider Certification for Patient Eligibility Form if they determine you would be anticipated to benefit from medical cannabis use.
Step 4: DOH Submission and Card Issuance
Once certified, we electronically submit your complete application package to the New Mexico Medical Cannabis Program.
The Department will process your application and, upon final approval, mail your official registry identification card directly to you. Your card will identify you as a qualified patient authorized for medical cannabis use.
